The CMS Medical and Public Health Social Workers manage a combined caseload of children with chronic medical condition (and families).
Clients are seen from the Lea County and surrounding communities.
This position is mission critical as it assists in improving access to safety net services for children with chronic medical conditions in areas of the State that have limited health care services.

To provide direct client services to clientele.
Will provide services to multi-ethnic, mono-lingual population, and provide assurance of services for children and youth with special healthcare needs.
Services will be addressed on an individual, family, and systems level.
How does it get done?
Provide direct client services for medically and financially eligible clients to attain mutually determined goals; gather, generate and maintain appropriate records and accurate completion of forms for social work intervention, expand, enhance and contribute to the improvement of services of the Children's Medical Services program; develop and maintain personal skills knowledge and understanding of Public Health, Children's Medical Services and related information needed to assure accurate, competent, and professional implementation of the Children's Medical Service program and the Public Health Division's Mission statement.

Ideal Candidate
The Ideal Candidate will be a Licensed Social Worker; have experience with case management; good communication skills; and the ability to multi-task.
Minimum Qualification
Bachelor's degree in Social Work (in accordance with NMAC 61-31-9) and two (2) years of experience in a social work setting such as social service agencies, healthcare, medical, educational settings, mental health, home health, hospice, developmental disabilities and other relevant experience, working with patients, clients, providers and families.

Employment Requirements
Must possess and maintain a valid driver's license.
Must be a Licensed Social Worker in the State of NM with a LBSW, LMSW, LCSW or LISW.
Working Conditions
Services are provided in communities, clinics, auxiliary site settings and in the homes of consumers to a diverse racial and ethnic population, many of whom do not speak English.
Reaching and lifting up to fifty pounds may be required.
Potential exposure to communicable diseases.
Travel to provide outreach and coverage to attend meetings and training sessions with occasional overnight stay is required.
