QUALIFICATIONS
• Graduate of an approved Licensed Practical/Vocational Nurse program and licensed in the state of practice, required.
• One-year experience providing specialized treatments in long term care setting.
• Minimum of 2 years of nursing experience in a Skilled Nursing Facility preferred.
• Current BLS/CPR certification (All SNFs; ALFs as required by state)
RESPONSIBILITIES.
• Perform resident skin assessments, obtain treatment orders, administer treatment as ordered, and document appropriately to ensure that resident skin integrity is improved and/or maintained.
• Documents findings appropriately and contacts physician to make recommendations for new or revised treatment orders; Monitors the progress of wound healing and makes recommendations for changes as appropriate.

• Maintain records of pressure ulcers and skin alterations including risk assessment, prevention and minimizing activities; Performs and records weekly pressure ulcer assessment and measurement documentation; Identify and maintain effective methods to gather pressure ulcer and skin alteration related information; Calculates weekly / monthly pressure ulcer rates.
• Investigate and identify center practices and risk minimizing measures, including accurate completion of Braden risk scales on admission, implementation of preventive interventions.
• Monitor timely implementation of preventive interventions for at risk residents, including new admissions, individuals with skin alterations and individuals at risk for development of skin alterations.
• Educate newly hired nursing staff on Skin Practice Guide requirements and Skin Resource Library including wound assessments, preventive interventions, care planning, treatment selection options, documentation requirements and education resources available.
• Evaluate availability of needed supplies for wound care and coordinate ordering with central supply.
• Assist the Director of Nursing and medical director with case review for center acquired pressure ulcers.
• Monitor changes in state and federal regulations and current industry-standard practices; Keeps current with industry trends through ongoing specialized education in the area of skin and wound management
• Provide monthly, quarterly and annual reports including recommendations for changes in center practice for the Quality Assurance and Performance Improvement Committee.
